We participate with many managed care plans. The key to smooth processing of these claims is the Referral Form from your Primary Care Physician. We welcome Medicare patients and are pleased to submit your Medicare and any secondary insurance claims.
Please note that our policy requires payment in full at the time of treatment. If you are not covered by an insurance plan with which we participate, please understand that you are ultimately responsible for payment.
